Contest Objectives: How does it work?
To fulfill contest requirements each team member must: • Commit to aerobic exercise (swimming, walking, jogging, etc.) for 30 minutes per day, 5 days per week for 10 weeks. • Set a personal weight loss goal between 0-20 lbs. (Weight loss is not required, you may only want to maintain your current weight. • Team captains coordinate a confidential weekly weigh-in for all team members and reports weight and exercise accomplishments to Partnership weekly by fax • Captains who submit paperwork on time win extra points for their team • Chance to win weekly prizes if exercise goal is met and captain submits paperwork on time • Individual and team prizes are awarded at the end of the contest if weight and exercise goals are met and if paperwork has been submitted on time throughout the contest How are teams made up? • Anyone 12 and older may participate.
• Choose co-workers, friends/family members
• Each team must have between 4-10 participants
• Promote your team with a unique name that best describes your team's personality, goals, etc.
• Each team is led by a Team Captain who will receive contest direction and rules prior to the start of the program.
Responsibilities of the Team Captain include:
• Motivate your team members
• Coordinate weekly weigh-ins (maintaining confidentiality)
• Submit team's weekly exercise/weight reports to Partnership
• Distribute weekly newsletters to team members
How much does it cost? $10.00 per participant. Team Captain participates FREE! Who can participate? Anyone age 12 and older. Students will need their parent's permission. How do I get started?

• Payment $10 per participant (Captain participates free) • Fax forms Liability Release/Initial Weight Report to Partnership by January 3, 2008 • Mail or bring payment to Partnership (Please make checks available to Partnership for a Healthier Johnson County).
Start your first full week of exercise (January 6 - January 12, 2008)
• Due January 16, 2008
• Complete team results on the Weekly Weigh-In/Exercise Form and fax to Partnership (317) 346-3738
• Due dates for each week following are listed at the bottom of each column on the form
• It is recommended that each team weigh in Monday prior to the Wednesday due date. • Remember to refer to the Contest Rules Brochure in the Registration Kit to answer most questions including how to report when team members are on vacations, sick, etc. • Questions: Call 346-3728 | |||||
